Payment Options
The Agape
Center accepts many forms of payment for the convenience of our patients.
All major credit cards are honored
(VISA, MasterCard, American Express, and
Discover) as well as personal checks and money orders.
We offer an affordable
no interest payment plans up to 18 months and extended
payment plans up to 48 months via CareCredit. To apply,

your status.

easier and more affordable than ever to pay for larger
purchases. Use this

practical plan gives you a variety of payment options and terms to choose from.
Each one is designed to meet your financial
situation. The plan also includes:

what costs will be)
• No payments for up to 18 months

Sight
for Students is another program that our office participates in. This
program provides free eye exams, glasses and vision therapy for the uninsured
working poor. To qualify please contact your local YMCA, Boys & Girls Club
or Head Start in your area. They will determine eligibility and refer you
to our office. Click

We
are also members of the Children's Resource Program of Ventura County.
This is a program of Interface and through it
we donate our time, expertise and services to you. To qualify for this program you should call (805) 485 6114. Click on

Our
office also participates in the
California Vision
Foundation. This organization, coordinated with the California
Optometric Association, helps to provided vision exams for the uninsured working
poor. We provide pro-bono services and optical laboratories provide
glasses. To be eligible for an
exam through the California Vision Foundation, patients must be employed -or
live in a household where someone is employed, must qualify as low-income (based
on a sliding scale, which includes number of household members and gross annual
income), must not have had an eye exam in the past two years, and must not have
any insurance (public or private) that covers vision services.

Our office
also provides vision screenings to the
Conejo Valley Neighborhood for Learning (NFL) program. Contact

provide pro-bono services for the
Conejo Free Clinic.

Several local
Lion's Clubs have also
financially helped low-income families with eye exams, glasses and vision
